Health Visiting Service 0-5 years

The Health Visiting Team are here to support you and your family up until your child is 5 years of age. We deliver the Healthy Child Programme which provides health advice and parenting support for you and your child. Universal offer to every family across Somerset your Health Visitor will visit on the below 5 core contacts:

  • Antenatal contact: from 28 weeks of pregnancy
  • Primary birth contact: between day 10-14
  • 6-8 week review contact
  • 9-12 month review contact
  • 2-2.5 years review contact

Additional support is provided to children and families who may need additional support. We also welcome everyone to our Healthy Little Ones which is a friendly drop-in group run by the Health Visiting team, where you can access free health and development advice, practical and emotional infant feeding support, weigh your baby and an opportunity to meet other families.

Fun Through Food and Movement group is also available which is a friendly drop-in session where you can meet other families, participate in games, join in with movement activities, get involved in food preparation and help cook nutritious meals and snacks to take home.

Chathealth: For a safe and easy way to contact the Health Visiting Team we offer the Chathealth service, where you can send a text for confidential advice and guidance from the Health Visitor. Send a text to 07480635514.

Self-Weigh: Self weigh at local libraries within Somerset please visit our social media page for further information and poster.

FOREST Programme: Giving Families opportunities and Resources for Early Years Support, together. A community-based programme offering enhanced parenting support alongside The Healthy Child Programme. Please see link below and further discuss referral to Families Opportunities Resources Early years Support Together (FOREST) with your Midwife or Health Visitor.

School Nursing Information 5-19 years

School Nurses provide a vital and unique link between school, home and community. Using Public Health approach and through offering early identification and intervention through:

  • Focusing on prevention and promoting health, well-being and protection for children and young people in all settings throughout their school aged years. Somerset children and young people website.
  • Identify the health needs of individuals and communities using evidence-based assessment tools.
  • School Nurses champion children and young people to reach their full health and wellbeing potential.
  • Follows the Healthy Child Programme 5-19
