I Offer to Assist/Support adults within Yeovil (ideally walking distance of Yeovil College area – BA21 4AE) “in as sustainable way as possible”– eg recycling, walking, saving water etc. Covid – I have had 2 vaccinations, the Covid booster and this years flu jab. My assistance/support covers Inside Activities of Daily Living using a mask & gloves, and Outside Activities eg gardening, walking, swimming, golf (Crossing roads, swimming, golf etc require extra assessment & any expenses for me eg golf, needs to be paid for in advance. ) I will be available for at least 40 weeks a year being Yeovil College/Schools opening weeks. If I am unable to work (either because of planned holidays etc or at short notice, eg sickness, then I will aim to work on another day if agreeable. Or if a substitute person has already been found & agreed to between us, I will contact both of you asap for you to arrange cover. contacting me at 11am on that day only, by text. I will supply my Enhanced DBS certificate, dated 21. 10. 21 . I will supply my FISH Carer/Personal Assistant Certificate of Insurance dated 4. 11. 21I am a Micro Provider for Somerset CC, working as Self Employed. This means I will manage my NI & Tax. A “Written Contract/Service Agreement, with a Cancellation Notice” will be drawn up between you and I, based upon your Current Support Plan. I will supply an invoice monthly for hours worked, which needs to be paid from your Direct Payment Account within 7 days. Along with the weekly/monthly invoice, I will include this message “Please write/email me with whats going well, & for the future, is there anything that needs to be done differently, with details. ” be done differently, with details. ” I will keep all information and data between us confidential, held on my computer with passwords, firewalls and encryption, used by me only, and will only share when required by law/appropriate to Safeguarding.
