Services I Provide: A one- or two-hour support, but can work longer if necessary. I support with shopping, cooking, light housework, trips to the Doctors or hospital, sitting service. I am a caring and responsible person who cares very deeply about the care and support that people receive. I take the responsibility seriously. I am in very close contact with other support workers in the area, so if I am on holiday or sickness, I can recommend one of the others to take my place if required, until I return to work. My First Visit: On my first visit I will do an assessment to know the needs, how much support is needed, what is required of me which can be assessed as we go, and basically to get to know one another. Any information I have is under lock and key with only myself to access, I do a review every 6 months to make sure there are no changes, but this can change weekly, so the support will be adapted to suit. Terms and Conditions: Sometimes a cancellation is unavoidable due to illness, but a 48 hours’ notice is normally required. If I have to change an appointment, I will reorganise it around the client and their family’s needs. The cost of 1 hour support is £16, 1/2 hr support £10, payment can be paid weekly by cash, cheque, or paid by bacs. A receipt will always be given. A £00. 30 per mile will also be added if a long journey is needed. I am happy to accompany clients to evening functions or late appointments. A name and telephone number is required of a close family member or friend so that I can contact them in case of any problems or to update on any changes of my services. No personal care will be given, (only in an emergency). Any ongoing illnesses I would like to be made aware of. I would require one week’s notice if my services are no longer required, either by telephone, Email or letter.
