Description

My name is Jo Millbrook. I have worked for Somerset County Council in social care for nearly 20 years and qualified as a social worker in 2018. I spent 18 years of those working with the children and families section and worked in various departments including Looked After Children, Contact Supervisor, Safeguarding and also worked as a support worker in a residential setting.

I transferred to Adult Social Care in 2021 where I worked out of both Yeovil District, and Bath Royal United hospitals. In these acute hospitals I worked with the discharge team, aiming to get those medically fit either back home with a package of care or into a residential setting.

I am approachable, honest and motivated to support you and your family and am happy to work together to create a support plan that provides you with the support you require. My services include general home and community support.

This includes:

  • Service user led activities
  • Conversation and companionship
  • Trips out
  • Support with various appointments
  • Cooking and meal preparation
  • Shopping, with or on behalf of the client
  • Support with dog walks and pet care
  • Sitting service
  • Running errands as well as light domestic and gardening duties, if equipment supplied

Whilst being a micro provider my work has included:

  • supporting young adults with learning needs
  • supporting adults living at home with dementia and working closely with a physiotherapist as a rehab assistant and personal assistant to a client with paraplegia
