Description
South West Care Services provides domiciliary and live-in care delivering the regulated activity of Personal Care to adults aged 18 years and over living in their own homes within the community, including older adults and people with dementia, mental health needs, physical disabilities, autism and learning disabilities. Support is person-centred and outcome-focused, assisting with washing, bathing, dressing, continence care, mobility and medication, alongside help with meal preparation, daily living tasks, maintaining routines, appointments and safety in the home, while encouraging individuals to do as much as possible independently. For autistic people and people with learning disabilities we provide structured, predictable support using clear communication approaches, positive behaviour support and sensory awareness to reduce distress and build confidence. For people living with dementia or mental health needs we provide reassurance, consistency and routine-based support to promote wellbeing and reduce anxiety. Individuals are also supported to access community activities, maintain relationships and remain active in daily life. Care is planned through assessments, risk management and Mental Capacity Act principles, working with families and professionals to enable people to remain safely in their own homes and avoid unnecessary hospital or residential admission while maintaining dignity, choice and control.
